 Invisible Conditions Alert Program

Under the Invisible Condition Alert Program, specified by Utah Code 58-1-604, you have the right to request a decal placed on your driver’s license, ID card, or vehicle’s record. 

Please note that as therapists, under this law, we are only permitted to diagnose the following conditions currently specified by the code: Alzheimer’s Disease or Dementia, Autism Spectrum Disorder, Down Syndrome, Post Traumatic Stress Disorder, Schizophrenia, and/or any diagnosis that you and/or your therapist believes would impact your ability to communicate with first responders in the case of an emergency. 

Early identification of an invisible condition during interactions allows first responders such as law enforcement, firefighters, or emergency medical personnel to approach situations with heightened awareness and respond appropriately. If this is something you would like to explore with your therapist, please let them know directly. 

The process you will need to go through to qualify for this change to your driver’s license, ID card, or vehicle’s record is as follows: 

  1. Go through any formal or informal assessment process your therapist sees fit to best determine a formal diagnosis for a qualifying mental health condition. 

  2. If your therapist feels you meet criteria for a qualifying mental health condition, you and your therapist must then fill out this form to have the condition specified on your driver’s license or ID: https://dld.utah.gov/wp-content/uploads/sites/17/2022/10/Invisible-Condition-Request.pdf AND/OR this form to have the condition added to your vehicle’s record: https://tax.utah.gov/forms/current/tc-894.pdf

  3. You then send the form(s) to the appropriate agency(ies) using the contact information shown at the top of each of the linked to documents above. 

  4. The agency(ies) you submitted your form(s) to will then contact you with more information. It is recommended that you follow up with them as needed. 

  5.
